GRAFTS USED IN ANTERIOR CRUCIATE LIGAMENT SURGERY AND

Rupture of anterior cruciate ligament (ACL) is the most frequently encountered knee injury, and reconstruction of this ligament has great significance since it causes knee instability, meniscus injuries, development of early degenerative changes. Today graft to be selected, their fixation methods, methods that shall improve healing and graft-tunnel match are the issues most discussed. Allografts, autografts and synthetic grafts are mostly utilized in the reconstruction of ACL. Ideal graft to be used in the reconstruction of ACL should be applied easily, have low donor field morbidity, and can be applicable to normal anatomic localization of ACL, its rigid fixation should be done readily, it should be revascularized rapidly in the place of fixation, and it should gain mechanical and ultrastructural properties of ACL after the operation. In the study; semitendinosus and gracilis autograft was used in 62 patients, patellar tendon autograft in 28 patients and bone-patellar tendon allograft was used in 16 patients in the arthroscopic ACL reconstruction surgery applied to a total of 106 patients. Returning back to active life was succeeded in 3 months for the patients with patellar tendon autograft, in 4.3 months for patients with semitendinosus and gracilis autograft, and in 5.4 months for patients with allograft. As a result of 5-year follow-up; rupture in 5 patients out of 62 patients with semitendinosus and gracilis autograft, rupture in 3 and anterior knee pain in 16 out of 28 patients with patellar tendon, and rupture and various joint problems were detected in 7 out of 16 patients to whom allograft was applied.

ÖN ÇAPRAZ BAĞ CERRAHİSİNDE KULLANILAN GREFTLER VE UYGULAMA SONUÇLARININ 5 YILLIK SÜREÇTE DEĞERLENDİRİLMESİ

ÖÇB yırtıkları en sık görülen diz yaralanmaları arasındadır ve diz instabilitesine, menisküs yaralanmalarına, erken dejeneratif değişiklerin gelişmesine neden olduğundan bu bağın rekonstrüksiyonu önem taşımaktadır. Günümüzde rekonstrüksiyonda seçilecek greft, tespit yöntemleri, greft tünel uyumunu ve iyileşmesini artıracak yöntemler en çok tartışılan konulardır. ÖÇB rekonstrüksiyonunda allogreftler, otogreftler ve sentetik greftler kulanılır. Ön Çapraz Bağ (ÖÇB) rekonstrüksiyonunda kullanılacak ideal greftin; kolay uygulanabilir, düşük donör saha morbiditesi, ÖÇB"ın normal anatomik yerleşimine uygulanabilir olması, hemen rijit fiksasyonu yapılabilir olması, fiksasyon yerinde hızlı revaskülarize olabilmesi, işlem sonrasında normal ÖÇB mekanik ve ultrastrüktürel özelliklerini kazanması gereklidir. Çalışmada; toplam 106 hastaya uygulanan artroskopik ÖÇB rekonstrüksiyonu ameliyatında, 62 hastaya semitendinosus ve gracilis otogrefti, 28 hastamıza patellar tendon otogrefti ve 16 hastamıza iki ucu kemikli patellar tendon allogrefti kullanıldı. Patellar tendon otogreftli hastalarda 3 ayda, semitendinosus ve gracilis otogreftli hastalarda 4,3 ayda ve allogreftli hastalarda 5,4 ayda aktif yaşama dönüş sağlandı. 5 yıllık takip sonucunda; semitendinosus ve gracilis otogreftli 62 hastanın 5 inde kopma, 28 patellar tendonlu hastaların 3 ünde kopma ve 16 sında diz önü ağrısı, allogreft uygulanan 16 hastanın 7 sinde kopma ve çeşitli eklem sorunları tespit edildi.
